Integrated Care for Older People (ICOPE) program
The Integrated Care for Older People (ICOPE) program is an innovative health promotion approach developed by the World Health Organization (WHO). This program aims to support healthy aging by focusing on prevention, proactive assessment of needs, and personalized support for older adults in order to preserve their autonomy for as long as possible.
About ICOPE
The Integrated Care for Older People (ICOPE) was designed and is recommended by the World Health Organization (WHO) to address the challenges of population aging.
It focuses on monitoring the 6 main intrinsic capacities to detect early signs of decline and support timely prevention. It is an international program implemented in multiple countries.
The program is designed for:

Older adults aged 60 and over, living at home or in a community setting, who wish to preserve their autonomy and quality of life.

Caregivers, who are actively involved in the ICOPE approach, especially to support screening, daily assistance, and access to resources.
In Quebec, the program has been adapted to fit the province’s cultural, linguistic, and organizational context while remaining aligned with WHO’s core principles.

Intrinsic Capacity: The Core of the ICOPE Approach
The central pillar of the ICOPE program is the monitoring of intrinsic capacity, defined by the WHO as the set of physical and mental abilities essential for maintaining autonomy.

Mobility
The ability to move independently within one’s environment
Cognition
The ability to reason, remember, and make decisions
Nutrition
The ability to eat well, maintain a healthy weight, and avoid malnutrition
Vision
The ability to see clearly, even with corrective lenses
Hearing
The ability to hear and participate in a normal conversation
Psychological Wellbeing
Presence or absence of symptoms such as depression, anxiety, or lack of interest.
How it works
Step 1
Screening
Using a smartphone or computer, individuals complete a quick and simple screening of their key capacities, either independently or with assistance.
Step 2
Assessment
If the initial screening suggests it is needed, a more in-depth assessment will be recommended.
Step 3
Recommendations
Based on the findings, tailored recommendations and interventions are provided.
Step 4
Follow Up
Individuals reassess themselves every six to twelve months to monitor changes over time.
